  • SUMMARY:Within the context of an innovative client centered care model, and in accordance with the standards of practice and the established vision and values of the organization, the Care Coordinator works as part of a multidisciplinary community team using a strengths-based model, comprehensive care plans and discharge planning services to support clients with mental health and substance use concerns. Provides care coordination services including crisis intervention and group facilitation, establishes and maintains relationships with clients, collaborates with multidisciplinary team and other community resources, liaises and consults with internal/external physicians, governmental and community social service, health and mental health agencies in facilitating clients’ access to services. This work is framed within VCH’s commitments to our pillars of Indigenous Cultural Safety, Anti-Racism, Equity, Diversity and Inclusion, and Planetary Health.QUALIFICATIONS:Education & Experience Bachelor’s degree in a behavioural science discipline from a recognized program. Two (2) years’ recent related experience or an equivalent combination of education, training or experience. Valid BC Driver’s License. Local area travel may require the use of a personal vehicle. Knowledge, Skills & Abilities Knowledge of the principles of recovery in mental illness. Broad knowledge of substance use issues, treatment, and the recovery process. Broad knowledge of the application of a Strengths-Based approach in Mental Health and Substance Use. Demonstrated ability to collaboratively develop SMART Goals with client as they apply to client-centred care planning. Comprehensive knowledge of Trauma Informed Practice, Cultural Safety, Harm Reduction and Recovery Oriented Care. Demonstrated knowledge of the impact of colonization on Indigenous peoples. Broad knowledge of behaviour management and ability to utilize non-violent crisis intervention strategies in a trauma informed manner. Knowledge of the difference between psychotherapy and basic counselling/wellness and coping oriented conversational skills. Demonstrated ability to provide basic counselling/wellness and coping oriented conversational skills. Broad knowledge of community resources. Broad knowledge of housing issues, both locally and provincially. Broad knowledge of legislation and healthcare organizational policies and procedures as it relates to the rights and obligations of clients and staff. Demonstrated ability to conduct bio-psychosocial, cultural, spiritual, and substance use screeners. Demonstrated ability to communicate effectively in a culturally safe manner, both orally and in writing, with clients, colleagues, physicians, and other health care staff, both individually and in groups. Demonstrated ability to establish rapport with clients experiencing multiple barriers. Demonstrated ability to build and maintain relationships with a variety of individuals/service providers. Demonstrated ability to work both independently and collaboratively as a member of an interdisciplinary team. Broad knowledge of other health disciplines and their role in client care. Demonstrated ability to adjust to unexpected events, analyze and problem-solve, set priorities, deal effectively with conflict, and respond to crisis. Demonstrated ability to establish workload priorities in collaboration with others. Physical ability to perform the duties of the position. Basic computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet and e-mail software.The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check. Details The Care Coordinator is part of HOPE (Housing Outreach Preventing Eviction), a team that supports clients in Vancouver to maintain their housing. Working within the context of an integrated team-based care model, the Care Coordinator is responsible for coordinating the care of clients that are attached to the HOPE Program and require household management support as a key component of their care plan. HOPE focuses on offering a short-term intervention based on intensive deep-cleaning and/or household management supports that address immediate risks of eviction, life safety issues, or barriers to discharge from hospital.
