SUMMARY:Within the context of a dynamic client and family centred care model and in accordance with the established mission, vision, values and objectives of the organization, works under the general operational supervision of the Patient Services Manager, clinical supervision from the Practice Coordinator and receives work direction from Social Workers in a team based environment and provides social work support services during the admission and discharge of patients/clients. Duties include interviewing clients/families/facilities for information, assisting clients to approach external resources, providing practical support to clients regarding benefits, housing, transportation, community resources, finances, and acquiring and maintaining a supply of up to date information and applications on services/agencies/resources.QUALIFICATIONS:Education & Experience Completion of a recognized Social Service Worker or Social Service Diploma Program or an equivalent combination of education, training and experience.Knowledge, Skills & AbilitiesAbility to communicate effectively both verbally and in writing.Ability to deal with others effectively.Physical ability to carry out the duties of the position.Ability to priorize and organize work.Ability to operate related equipment.The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
