SUMMARY:Within the context of a client centered care model and in accordance with established standards of professional practice and the vision and values of the organization, works as part of an interdisciplinary community team to provide clinical and consultative social work services to individuals with chronic health and mental health issues, poverty and victims of violence. Performs professional social work services such as conducting comprehensive social and psychosocial assessments, preparing comprehensive case histories, providing psycho-education and interventions, establishing, maintaining and enhancing therapeutic relationships, evaluating clients’ response to treatment plan and coordinating individualized goal-oriented treatment. Develops and enhances community communications/relationships, provides consultation services, engages in on-going team planning, development, evaluation and quality improvement activities to achieve excellence in client care. May act as a Designated Responder under the Adult Guardianship Act.QUALIFICATIONS:Education & Experience Current full registration with the British Columbia College of Social Workers.Master’s degree in Social Work from an accredited university. Two (2) years’ recent related experience working with individuals with mental health issues in a health care setting, or an equivalent combination of education, training and experience. Valid BC Driver’s License and access to a personal vehicle for business-related purposes. Details In this role, the Social Work clinician will focus on screening of incoming referrals as well as clinical assessment of clients to determine necessary clinical supports and appropriate housing model based on need, housing policy and best practice. The clinician works closely with various partners including mental health teams, tertiary sites, and housing partners to support safe and sustainable placements into supported housing site.
