Acts as the primary point of contact for Indigenous children, families, and healthcare providers to provide support and advocacy. Develops and documents coordinated service plans including assessments, referrals, and ongoing care protocols.
Requires a diploma in a Human Services related field and three years of recent experience in health and social services, specifically with Indigenous Peoples. A bachelor's degree in health or social services is preferred.

The target hiring range for this position is $38.0337 to $46.7918 per hour. Candidates will typically be hired, transferred or promoted between the minimum and midpoint of the wage range. Placement within the wage range is dependent on the applicants’ role specific knowledge, skills, abilities, related experience and in consideration of factors such as market conditions and internal equity.
