The In-Reach Worker provides one-on-one support to patients experiencing homelessness to facilitate system navigation and discharge planning from hospital services. This includes coordinating with hospital teams and community partners to secure housing and financial supports.
Candidates must have a Bachelor of Social Work degree or equivalent and experience working with vulnerable populations. A valid driver's license, reliable vehicle, and certifications in CPI and First Aid/CPR are required.

Blue Door is a not-for-profit registered charitable agency governed by a volunteer Board of Directors. We have been an integral part of our community providing essential emergency shelter services since 1982. Blue Door’s mission is to support people who are at risk of or experiencing homelessness to attain and retain affordable housing.
Reporting to the Program Manager, Transitional and Health, the Hospital In-Reach Worker (Case Manager) is a social worker or equivalent, supports individuals experiencing homelessness accessing hospital services (Emergency Department and In-Patient Unit) with system navigation and discharge planning. This worker works both remotely and on-site at Southlake Hospital as directed.
