The pharmacist is accountable for patient pharmacotherapy and the provision of comprehensive pharmaceutical care within the Renal Clinic. Responsibilities include clinical dosing, collaborating with multidisciplinary teams, and delivering teaching to pharmacy residents and students.
Candidates must hold a Baccalaureate Degree in Pharmacy or Pharm D and be licensed to practice in Ontario. Experience in critical care, hospital pharmacy residency, and providing patient-centered care is preferred.
Reporting to the Clinical Manager of Pharmacy Services you will be an integral member of the Pharmacy team and be accountable for the pharmacotherapy for patients to ensure optimal patient care is achieved. You will be evaluating evidence-based medicine to support your recommendations and to develop improvement strategies for drug utilization. Key Qualifications: Baccalaureate Degree in Pharmacy or Pharm D Licensed in Ontario to practice pharmacy Completion of hospital pharmacy residency or equivalent experience is an asset Critical Care experience preferred Strong interpersonal, problem solving, organizational, communication, documentation and critical thinking skills Experience in providing exceptional patient centred care Experience in hospital pharmacy systems and distribution systems are an asset. Past experience or a keen interest in teaching and research activities is an asset Relevant clinical experience preferred Ability to work in a fast paced environment both independently and cooperatively with the team Demonstrated self-directed learning and continuing education Positive attitude, flexibility to adapt to changing organization needs and accountability Responsibilities Include: Provision of comprehensive pharmaceutical care to patients Clinical care of patients in comprehensive clinical programs including warfarin dosing, IV to PO, aminoglycoside and vancomycin dosing. Use of Pharmacists' clinical scope of practice as per THP's Policy and Procedure to optimize patient care In collaboration with other clinical pharmacists deliver appropriate teaching to pharmacy residents and pharmacy students Participate in pharmacy case presentations, Pharmacy Education Rounds, journal clubs, case discussions and research projects Drug distribution responsibilities. Collaborate with physicians, nurses and allied health team members to provide optimized patient care Other duties as assigned including the ability to potentially work at all Trillium Health Partners sites Must be able to provide coverage on various clinical units as needed THP is committed to internal talent, and therefore priority will be given to internal applicants. Your primary site initially will be Mississauga Hospital; however, you may periodically be required to work at another THP site and therefore periodic travel between sites may be necessary.
