Assess nutritional status and develop individualized, culturally appropriate care plans aligned with client needs and resources.
Provide nutrition counselling, education, and training on meal planning, food safety, and adherence to care plans.
Monitor client progress using weight, labs, and outcomes, and adjust plans as needed.
Collaborate with healthcare and community partners, including referrals, advocacy, and resource coordination.
Maintain professional, ethical, and safe practice by adhering to privacy, consent, health, and safety standards.
You are registered as a member in good standing with the College of Dietitians of Alberta.
Familiarity with tube feeds would be considered an asset.
Your focus is to embrace person and family-centered care.
You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions.
You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently.
You value the role a dietitian plays in keeping clients at home when that is of personal importance.
You have the ability to travel within community as part of the CBI Health Home Health Services team.
