Bayshore Medical Personnel is seeking a full-time RN Case Manager (Care Coordinator) in Halifax, NS to provide case management service to our client’s members who suffer from protracted and complex health issues.
RN Case Manager’s work as a member of a multidisciplinary primary health care team in a primary care/clinic setting. Their primary responsibility is to complete detailed case management assessments, develop comprehensive care plans that reflect individual health care needs, and monitor clients and make suggestions for modifications as necessary.
Successful candidates will possess 3 full-time equivalent years of clinical experience within the last 4 years in nursing practice, including previous case management experience.
Candidates must have a Baccalaureate Degree in Nursing, a current provincial license, and BLS or CPR/AED certification. At least 3 full-time equivalent years of clinical experience within the last 4 years, including case management, is required.
