Provide one-to-one clinical home care including wound care, medication management, and palliative support for clients. Assess health status, coordinate with physicians, and document all clinical observations using the AlayaCare system.
Must be a Registered Practical Nurse in good standing with the College of Nurses of Ontario and possess a valid driver's license with a reliable vehicle. A Vulnerable Sector Check is required prior to starting the position.
Clinical scope and responsibilities follow College of Nurses of Ontario (CNO) standards of practice. Care is documented through AlayaCare — a structured electronic system that gives our nurses a defensible, professional clinical record for every visit.
