About the role

Provide client-centered clinical care and conduct home visits under the supervision of an Occupational Therapist or Physiotherapist. Responsibilities include supervising exercise programs, assisting with mobility and ADL training, and documenting client progress.

Pay Range

Base Salary

$53,000.00 - $66,000.00 (commensurate with experience)

We are seeking Occupational Therapy Assistant for our MGH2Home Program

This is a unique Occupational Therapy Assistant position supporting East Toronto Health Partners’ (ETHP) strategy to improve client transitions from hospital-to-home through the newly established MGH2Home program. This short-term transitional program (up to 90 days) involves linking together Michael Garron Hospital (MGH), Spectrum Health Care, VHA Home HealthCare and WoodGreen to support earlier discharges of adults with higher needs to reintegrate into their home and community. This multi-disciplinary team will work as “one team”, along with the client and designated family, to address their complex health and social needs in the development of a single “one team” care plan.

As an Occupational Therapy Assistant at VHA Home HealthCare you will provide client-centered clinical care under the direction and supervision of an Occupational Therapist/Physiotherapist. You will be conducting home visits for the clients of VHA as per the Occupational therapist's recommendations.

Areas of accountability

  • VHA's Occupational Therapy Assistants have the following key areas of accountability, but not limited to:
  • Assist/supervise the client with his/her individual prescribed exercise programs or ADL training
  • Assist/supervise client with walking, transfers, stairs, safe mobility, and positioning using proper body mechanics
  • Provide client/family education and support to address physical, functional, and self-care needs as assigned
  • Assist with gross motor, fine motor, and balance activities
  • Monitor and report changes in health status to the Physiotherapist and Occupational Therapist
  • Participate in program planning, quality improvement, and evaluation
  • Document in the client record and collect required data as requested

Required Qualifications

  • Diploma as an OTA/PTA from an accredited college program.
  • You have a minimum of 2 years relevant work experience, preferably in the community working with geriatric clients with variable diagnoses and multiple co-morbidities.
  • You are a skilled communicator and have a successful track record of coaching/mentoring clients to participate in a professional manner.
  • Current First Aid Certificate and Cardiopulmonary Resuscitation (CPR) Certificate required.
  • Evidence of continuous education.
  • Sound critical thinking, good judgment, and problem-solving skills.
  • The ability to work effectively and collaboratively with a multi-disciplinary team.
  • Demonstrated understanding and practice of rehabilitation principles and techniques.

Requirements
