Provide comprehensive medical management for patients in acute and subacute rehabilitation facilities. Collaborate with multidisciplinary teams to develop individualized care plans and prevent hospital readmissions.
Must hold an MD or DO degree and have completed an ACGME-accredited residency in Internal Medicine. Requires board certification or eligibility and a valid state medical license.
Provide comprehensive medical management for patients admitted to acute and subacute rehabilitation facilities. Conduct initial evaluations, comprehensive medical assessments, and ongoing follow-up visits to develop and oversee individualized plans of care. Diagnose, treat, and manage acute and chronic medical conditions, including complex comorbidities commonly encountered in post-acute care settings. Monitor patient progress, respond to changes in clinical status, and coordinate appropriate interventions to promote recovery and prevent avoidable hospital readmissions. Review laboratory results, diagnostic studies, medication regimens, and treatment plans to ensure safe, evidence-based medical management. Provide education and counseling to patients and families regarding diagnoses, treatment plans, medication management, and post-discharge care recommendations. Maintain accurate, timely, and compliant documentation within the electronic medical record. Adhere to all regulatory, quality, and compliance standards while contributing to facility performance initiatives and patient satisfaction goals.
Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school. Completion of ACGME-accredited residency program in Internal Medicine. Board certification or eligibility in Internal Medicine. Valid medical license in the state of practice. Strong clinical skills in the evaluation and management of patients with acute and subacute rehabilitation needs. Excellent interpersonal and communication skills with the ability to collaborate effectively within a multidisciplinary team. Maintain accurate and timely documentation of patient evaluations, progress notes, and discharge summaries. Demonstrated leadership abilities and a commitment to professional growth and development. Commitment to delivering compassionate and patient-centered care.
