About the role

The Nurse Practitioner provides primary health care including prevention, assessment, diagnosis, and treatment. They function independently within the Extended Class scope of practice while collaborating with an interprofessional team using wholistic health concepts.

EMPLOYMENT OPPORTUNITY Full-Time Existing Position

Nurse Practitioner Our teams provide primary health care through a range of options including prevention, assessment, diagnosis and treatment. Team members include health professionals, educators, and support workers representing both traditional Indigenous and contemporary models of care, and use wholistic health and community development concepts. The Nurse Practitioner functions independently within the Extended Class scope of practice, and in collaboration with members of the interprofessional team. NOTE: Traditional practices involving burning sacred medicines including tobacco, sweetgrass, sage and cedar and other cultural protocols occur regularly within our work setting.

Primary duties / Responsibilities

Provide comprehensive assessment, diagnosis, treatment, prescribing, ordering and interpretation of diagnostic investigations, and referral within the NP scope of practice. Coordinate and monitor client care plans to promote optimal health outcomes and continuity of care. Collaborate with professional team members to ensure comprehensive wholistic care. Provide clinical leadership, consultation, and mentorship to staff, students, and team members. Participate in the development, implementation, review, and evaluation of medical directives, clinical protocols, and evidence-informed practice standards. Contribute to quality improvement, risk management, program evaluation, and organizational learning activities. Support the development and evaluation of programs and initiatives that advance health promotion, disease prevention, and population health priorities. Maintain current and accurate medical records, reports and other documentation to ensure comprehensive, consistent care and services, and to meet all accountability requirements. Active participation in organizational teams and committees. Other duties as required. In extenuating situations or in the event of an emergency, may be redeployed to ensure provision of essential services.

Qualifications

current registration in the Extended Class with the College of Nurses of Ontario; demonstrated ability to work independently and be an effective team member; excellent interpersonal and communication skills demonstrated ability to communicate effectively with a broad range of people including healthcare professionals, traditional people, leadership, and community members; understanding of wholistic health and well being, demonstrated cultural competencies and experience working in an Indigenous organization; ability to understand and speak Anishinaabemowin a definite asset; willingness and ability to travel, with significant community-based activity; valid drivers’ license and own vehicle required; copy of current drivers’ abstract required; proof of immunization in compliance with policy requirements is mandatory; current criminal records check required; commitment to ongoing training and professional development relevant to job requirements; positive attitude and capacity to act as a healthy lifestyle role model.
