About this position: In this role, you will provide comprehensive psychosocial assessment, care planning, and treatment coordination for a designated client population. Working as part of a multidisciplinary team, you will collaborate closely with health care providers to deliver high-quality, integrated care. You will offer direct support through counseling, group facilitation, and psychosocial interventions, while helping clients and families navigate complex systems and access appropriate resources. Your ability to advocate, educate, and connect individuals with specialized services will empower clients to strengthen self-management and improve overall well-being. As a Social Worker, you will contribute to the development and delivery of social work services that enhance health outcomes and support the social functioning of the population you serve. Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
