About the role

The Care Coordinator is responsible for assessing, planning, and implementing individualized nursing care for residents in a memory care setting. This includes administering medications, collaborating with multidisciplinary teams, and communicating resident conditions to families and physicians.

Seasons Retirement Communities is seeking a compassionate, motivated individual to fill the role of Care Coordintor, RPN Memory Care.

This is an excellent opportunity for the successful candidate to provide professional nursing care to residents in accordance with required medical care and established nursing care plans and practices.

The successful candidate will be responsible for areas including

  • Assessing, planning, implementing and evaluating individualized care to meet the residents' physical and emotional needs. Ensure resident care plan is initiated in collaboration with other members of the multidisciplinary team
  • Communicate with relatives regarding the care and condition of respective residents
  • Liaise with attending physicians, psychiatrists, consultants, social workers, clergy, relatives or responsible persons, as well as other members of the Residents and Guest Services team to ensure effective resident care is provided
  • Assists and attends to care needs of the residents, including administration of medications as required
  • Maintains work and storage areas in a clean, safe and sanitary manner
  • Participates as a member of the Memory Care Services team and provides support to the other members of the Resident and Guest Attendant team
  • Observes and adheres to all policies and procedures, work standards and safety regulations
  • Is aware of employee’s and supervisor’s responsibilities and follows all health and safety policies and procedures
  • All other duties as assigned or consistent with work routines and job descriptions

Qualifications

  • Must be able to read and communicate effectively
  • Must be able to follow written and verbal instructions
  • Demonstrated empathy and understanding of the needs of seniors
  • Effective interpersonal relationship and conflict resolution skills
  • Ability to organize work and work under strict time constraints
  • Must have a high standard of hygiene and cleanliness
  • Must be able to operate related equipment
  • Must be able to respond to emergency situations
  • Must have current CPR & First Aid certificate
  • Must have a current Registration with the College of Nurses

In addition, the successful candidate will be required to promote high quality care while maintaining a safe and healthy environment for residents, resident families, visitors and staff.

Experience as an RPN registered with the College of Nurses and experience within the seniors housing industry are required. Experience on a memory/dementia care floor preferred.

All applicants must be legally entitled to work in Canada. As a condition of employment, a Criminal Record Search which may include a vulnerable sector screen, will be required.

Requirements

Candidates must be a Registered Practical Nurse (RPN) with current registration with the College of Nurses and valid CPR & First Aid certification. Experience in seniors housing is required, with a preference for those experienced in memory or dementia care.
