The Family Physician will build strong patient relationships while providing comprehensive acute, preventative, and chronic disease management. They will collaborate with a multidisciplinary team to deliver integrated care and support the transition toward an FHO+ model.
Candidates must be licensed to practice as a Family Physician in Ontario and be in good standing with the CPSO. A minimum of one year of experience is preferred, though new graduates are welcome to apply.

We’re growing our Family Medicine team and recruiting Family Physicians to join our North York clinic. This is a chance to build a long-term family practice with strong clinic support behind you, and to play a key role in growing longitudinal family medicine at this location as we work toward forming a new FHO+ model.
From day one, you will work closely with a multidisciplinary team to support patient-centered care. In the near term, the practice will focus on walk-in medicine as we build toward a FHO+ model, with a growing focus on longitudinal care as the model takes shape.
