Case Manager Hestia Homes

• Program Delivery & Client Support
• Support clients in setting and achieving goals related to housing, financial stability, and legal supports
• Deliver case management using a Housing First approach
• Advocate on behalf of client needs and goals
• Build and maintain strong relationships with community agencies and service providers
• Participate in case conferencing to coordinate effective service delivery
• Maintain accurate and timely client documentation and records
• Support or facilitate life skills and support groups for youth in Hestia Homes
• Stay informed on trends affecting youth experiencing low income, mental health challenges, and addictions
• Apply knowledge of trauma-informed, person-centered, harm reduction, and Housing First approaches

• Program & Organizational Support
• Maintain program statistics and support monthly and annual reporting
• Contribute to program evaluation processes (biannual evaluations)
• Assist in the development of new initiatives to enhance services
• Support onboarding and training of staff, practicum students, and volunteers
• Complete additional duties as assigned by the Program Manager
• Accurately track work hours using UKG mobile app (no missed punch-ins/outs)
• Submit expense claims accurately and on time

Qualifications

• Diploma (2-year) in Human Services OR a Bachelor’s degree in a related field
• Suicide Intervention training
• First Aid certification
• Minimum 1 year of related experience
• Valid driver’s license, reliable vehicle, and $2M liability insurance
