We are looking for awesome, self-driven nurses to work throughout the Northumberland Region to help ALC clients in their home environment.

Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
Administer medications orally, topically, subcutaneously, or via G-tube, as per CNO scope and documented medical orders; document all administrations in the electronic Medication Administration Record (eMAR).
Monitor and assess clients’ vital signs (blood pressure, pulse, temperature, respirations, oxygen saturation), document findings, and report any abnormal readings or concerns to the designated Care Coordinator or physician.

Support clients with medical needs such as oxygen therapy (monitoring levels, changing tanks, checking saturation), G-tube feeding (preparing and administering formula, flushing lines), and suctioning as per training and client-specific protocols.
Safely assist clients with mobility and transfers using mechanical lifts (Hoyer, sit-to-stand) or manual assistance, following prescribed mobility plans and ensuring proper body mechanics and safety procedures.
Perform prescribed range of motion (ROM) and physical therapy exercises under direction of a physiotherapist or care plan, and assist with ambulation inside the home or outdoors if required.
Observe and document any changes in the client’s physical, cognitive, or emotional condition, including changes in skin integrity, alertness, behaviour, and breathing patterns; escalate concerns as needed.
Maintain accurate and timely documentation of all care provided, including progress notes, incident reports, and communication logs, in compliance with company and regulatory standards.
Use and dispose of Personal Protective Equipment (PPE) such as gloves, masks, gowns, and eye protection as per infection prevention and control protocols and Public Health Ontario guidelines.
Comply with organizational policies regarding infection control, client safety, privacy, and professional conduct; report all hazards or incidents to the supervisor immediately.
The following Requirements are MANDATORY: Active registration and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN) Current CPR and First Aid certification, completed through a recognized training provider within the past 12 months.
Valid WHMIS 2015 certification, issued or renewed within the past 12 months.
Negative TB test (2-step or 1-step with prior documentation), completed within the past 12 months.
Proof of full COVID-19 vaccination, or exemption in accordance with the Ontario Human Rights Code.
Clear Criminal Record Check with Vulnerable Sector Screening, issued within the past 12 months.
Must be able to communicate effectively in English, both orally and in writing, for documentation and communication with clients and care teams.
Must be comfortable and capable of providing care to both male and female clients of varying ages and care needs.
Must have a valid driver's licence and reliable access to vehicle to complete daily tasks Ability to identify and report client care concerns to Care Coordinators when needed.
