Partners in Canadian Veterans Rehabilitation Services (PCVRS) provides comprehensive rehabilitation focused assessment and intervention for Canadian Veterans who are experiencing barriers transitioning to life after service due to service related injuries or illness. Using a biopsychosocial approach, the assessment and intervention services are performed by an interdisciplinary team of clinicians with expertise in the management of complex conditions such as chronic pain, persistent symptoms following head injury, MSK injuries, often with comorbid mental illness with presenting psychosocial barriers to recovery.
As part of the interdisciplinary team the Physician shall use evidence and measurement-based care to perform the following:
Complete a thorough assessment of presenting eligible health problems;
Perform medication review and identify drug therapy problems;
Clarify relevant medical issues including comorbid conditions;
Clarify diagnosis and prognosis related to eligible health problems;
Identify conditions that are not related to the eligible health conditions that if left unaddressed can hinder progress in rehabilitation
Identify relevant accommodations or medical restrictions;
Identify and provide rationale when additional specialized assessments are required to enable achievement of rehabilitation goals;
Collaborate with the interdisciplinary team on treatment recommendations based on PCVRS continuum of care with an emphasis on education, coping and self-management to improve function and recovery;
Collaborate with the Veteran, support person (if present), family member (if present) to promote understanding of relevant medical conditions and recommendations;
Communicate directly, through targeted outreach, with the Veteran’s primary health care provider to collaborate on rehabilitation care;
Care may be provided in-person or virtual;
Participate in de-brief discussions with the interdisciplinary team, when necessary.
Engage in time-bound targeted follow up care through a consultative model with Veterans receiving rehabilitation treatment within PCVRS;
Collaborate with PCVRS Medical Director(s) and engage in continuous quality improvement, learning and skills development;
