The Licensed Practical Nurse (LPN) Outreach works within a multidisciplinary mental health team, performing assessments and providing personal and nursing care to clients with mental health disorders both in their homes and the community. Responsibilities include administering medications, monitoring patient status, contributing to care plans, and assisting clients with daily living activities.
Candidates must have graduated from a recognized Practical Nursing program or possess equivalent experience, along with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM). A valid BC Driver's License, access to a personal vehicle, and completion or eligibility for MHSU Competency Education are also mandatory.

Reporting to the Program Coordinator or designate and under the direction of the team leader, the Licensed Practical Nurse (LPN) Outreach works as a member of the multidisciplinary mental health services team in a collaborative nursing model with RNs and RPNs. The LPN Outreach performs assessments and plans, implements and provides personal and nursing care services to clients, both in their homes and in the community, who have a variety of mental health disorders. The LPN Outreach operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM) and in conformance with established policies, procedures, standards and philosophies of Fraser Health.
