About the role

The RN Case Manager provides holistic patient case management and coordinates care within a multidisciplinary team at the Pinehouse Health Centre. They are responsible for disease management, acute condition assessment, health teaching, and community outreach.

The RN Case Manager will report to the Primary Health Nursing Coordinator and Manager of Primary Health Care.

This position will be an integral part of the Primary Care Health Team within the Pinehouse Health Centre.

Although the primary base of work will be in a primary care setting, some community outreach and programming will be required.

The RN Case Manager will also provide support in areas of public health and communicable disease management.

The focus of the RN Case Manager will be to offer patient case management through holistic care coordination within a multidisciplinary health care team.

The RN Case Manager will take a lead in fostering close therapeutic relationships between the primary care customer, their families, and the wider clinical team primarily through disease management and acute conditions which involve assessment, diagnosis, referral, health teaching, advocacy, and health promotion.

Experience

Minimum of three to five years of nursing experience with demonstrated clinical, administrative, organizational, excellent team player and communication skills.

Two years in a primary care nursing position is also preferred.

ACLS and PALS required, or must be completed within 6 months of hire.

Experience working in a cross-cultural setting is preferred

Knowledge and Abilities

Demonstrated ability to participate as a member of an interdisciplinary team.

Demonstrated ability to maintain confidentiality of information.

Demonstrated ability to organize time and daily tasks.

Demonstrated good communication skills.

Demonstrated ability to delegate appropriately.

Demonstrated understanding of scope of practice of individuals supervised.

Requirements

Candidates must be a graduate of an approved School of Nursing with three to five years of clinical experience and current registration with the CRNS or RPNAS. ACLS and PALS certifications are required or must be obtained within six months of hire.
