The Licensed Practical Nurse will participate in scheduled community visits to provide patient care within their scope of practice, utilizing appropriate assessment tools and executing/monitoring care plans. They must engage patients collaboratively, maintain trust, and update professional patient records related to encounters and plans.
Candidates must be registered with the NSCN as a Licensed Practical Nurse in good standing and possess a valid driver's license with access to a vehicle. A minimum of 2-4 years of experience in community care delivery is considered an asset, along with a willingness to travel and participate in required on-call coverage.
