About the
Program: Our Mobile Mental Health Team provides non-crisis, short-term care to individuals aged 16+ transitioning between acute episodes and long-term support. Services are primarily delivered in clients’ homes, with flexibility to meet in public spaces (e.g., libraries, community centers) or virtually, based on client preference.

Team &
Culture: You’ll be part of a small, collaborative team consisting of two RSWs, one RN, and one administrative assistant. The role emphasizes flexibility, adaptability, and teamwork, working closely with clients to determine the most appropriate setting and therapeutic approach.
Key
Responsibilities:
Conduct in-home and community-based client visits
Provide therapy and counselling using evidence-based practices
Assess mental health and risk, including suicidal, self-destructive, homicidal, and aggressive behaviors

Work alongside law enforcement, paramedic services, and other community services when needed

Apply knowledge of mental illness, cognitive functioning, substance use, and medical comorbidities
Understand psychotropic medications and identify when further evaluation is needed
Collect information from interviews and collateral sources
Perform additional duties as required
Qualifications:
Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
Minimum 2–3 years of experience in community mental health, counselling, or case management
Bachelor of Social Work (BSW) required; Master of Social Work (MSW) is considered an asset
Experience working in Huron and Perth Counties is considered an asset
Strong clinical assessment and interpersonal skills
Flexibility, adaptability, and teamwork skills
Valid Ontario Class G driver’s license (company vehicle provided for client visits)
Clear criminal record check required
Additional
Training considered an asset: Trauma-Informed Care, Mental Health First Aid, ASIST, Nonviolent Crisis Intervention
