Develop personalized, evidence-informed care plans to support patient rehabilitation and wellness goals. Collaborate with an interdisciplinary team to coordinate patient-centric care and achieve optimal clinical outcomes.
Candidates must hold an active license in good standing with their provincial regulatory college. The role prefers 1–2 years of outpatient clinic experience, though new graduates with provisional licenses are encouraged to apply.
