The Community Inclusion Worker ensures the rights and choices of the supported person are respected while facilitating their participation in community activities. They also document daily activities and health information, fostering positive relationships with families and professionals.
Candidates must have a Grade 12 education and a Community Support Worker Diploma or equivalent, along with a minimum of 2 years of experience in community social services or health care. Proficiency in basic American Sign Language and current certifications in First Aid/CPR and Food Safe are also required.
As a Community Inclusion Worker, you will assist a person who requests support to participate in personal, social, vocational, and volunteer opportunities within the community. The services are determined by the person and based on their specific goals and preferences. You’ll encourage and empower the person to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety. Your natural ability to care for others and work collaboratively towards a goal will allow you to influence your person’s success.
Schedule- 30 hours/week
