The Opportunity The Licensed Practical Nurse (LPN) acts as the key communication contact between management, residents and their families as well as contributing to patient care and safety. They play a critical role in ensuring the well-being of residents, monitoring their conditions, and providing compassionate and skilled care.

What You Will Do

Work as a member of the inter-professional team as described by our Professional Practice Model applying the person-centered approach to care.

Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.

Implement and evaluate the effectiveness of the care/service plans and make changes as required.

Assist with scheduling of resident's care conferences and participate in conferences where applicable.

Work closely with the Director of Care to facilitate resident's move in, move out and transfers.

Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards and policies and procedures.

What You Bring

At least 1 years’ experience in working with seniors.

Demonstrated excellence in dementia care and interpersonal skills.

Completion of an accredited License Practical Nurse education program.

Active or eligible for registration and practice permit with the provincial College of Licensed Practical Nurses.

Current Basic Cardiac Life Support – Health Care Provider.

Requirements

Candidates must have at least 1 year of experience working with seniors and demonstrate excellence in dementia care. Completion of an accredited LPN education program and current Basic Cardiac Life Support certification are required.
